Life / Accidental Death & Dismemberment Insurance¨ Employee coverage available to full time employees. ¨ $20,000 minimum -or- equivalent of one times annual salary coverage for employee - paid 100% by JFM. Increased coverage (buy-ups) available. ¨ Coverage available for eligible dependents: $5,000 for spouse; $2,000 per child. Employee contributions vary for buy-ups and dependent coverage. ¨ Effective on the 1st day of the month following 30 day waiting period. Tax Sheltered Annuity 403(b) Plan¨ Available to all employees. Enrollment available upon hire or anytime thereafter. Pre-tax contributions up to the IRS limit per year. JFM offers short-term counseling (up to 5 sessions) to all employees and family members. Vacation Time¨ 2 to 3 weeks - accrued from date of hire based on position.
